This can be used as a sample request to establish a closure school date for a school that is missing one.

These are student loan intake questionnaires developed by the Legal Aid Foundation of Los Angeles and NCLC. The questionnaires include important questions concerning the client, the school, and the loan, and contains questions geared to determining if the client is entitled to a statutory discharge of the student loan.

This manual is designed to be used much like the AWG Hardship Calculator, as a tool in helping Private Collection Agency perform their duties, and like any other tools there will be changes and improvements.
