This is an older version of a HUD form used to compute the operating fund Calculation and delta for public housing. This information is required by 24 C.F.R. pt. 990. HUD makes payments for operation of low-income housing projects to PHAs. The Operating Fund determines the amount of operating subsidy to be paid to PHAs. PHAs provide information on the Allowable Expense Level (AEL), Allowable Utility Expense Level (AUEL) and Other Costs for the major Operating Fund components.

This is a log of what communications involving the consumer should be privileged and not available to the defendant. It shows for each communication, the date, the author, the recipient, the information, the privilege asserted, and the basis for that privilege. The log is filled in, showing examples of privileged communication and use of the log.
This is a discovery plan in a FDCPA case where the consumer attorney lists the various elements to be proved in the case, what evidence is needed to prove that element, whether the consumer attorney has that evidence, and what evidence should be requested of the defendant or where the consumer attorney should seek that the defendant admit to that element.
